Programs that monitor and manage the administration of medication, principally warfarin (also called Coumadin), that is used to prevent and treat blood clots that can cause a stroke, heart attack or other serious medical problem. Anticoagulants are prescribed for a variety of conditions, and periodic monitoring including regular blood tests are often necessary to make sure that patient's condition is being treated appropriately and safely. Clinic staff also assess for any changes in the patient's medications and diet, as well as any side effects related to the therapy.
