Photo ID; proof of income; court documentation if necessary
Eligibility
Anyone with a need
Fees
$50 evaluation fee; $30 drug tests. For classes, accepts sliding scale, some insurance, Medicaid (Integral, Prestige, Molina or Cenpatico [Sunshine Health]). Does not take Medicare.
