Mammogram prescriptions from healthcare provider (must be attached to completed application), proof of residency (attach copy of FL drivers license & Fl voter registration card); one application per 12 month period
Eligibility
Full-time Resident (female/male) within zip codes serviced by program, 40+ or medically necessary at younger age, and must have financial need based on combined yearly household income.
