Eligibility
An initial health care assessment is done to ensure eligibility.
Fees
Private pay: $11-$13 /hr (no minimum); Medicaid: fees covered by third-party insurer Long term care insurance and Veterans Benefits

Call or fill out contact form on website; meeting is scheduled with the director or other staff member to go through the intake process together.
